Financial Responsibility
I understand that, by indicating my agreement and submitting this registration, I am responsible for all charges incurred by my pet while in the care of the doctors at Rock Bridge Animal Hospital. Charges are due and payable at the time of service unless other arrangements are made in advance. Any balance carried over a period of 30 days will accrue a monthly finance charge of 1.5% (18% per annum). Any balance left unpaid may be forwarded to Rock Bridge Animal Hospital’s collection agency and will incur a $35 collection fee, in addition to any applicable finance charges, for which I am liable.
Appointment Booking Fee
A $55 booking fee will be collected to hold my appointment spot. This fee is NON-refundable if I cancel my appointment with less than 24 hours’ notice. If the appointment is kept as scheduled, this fee will be applied toward my invoice. Appointments may be canceled by calling (573) 859-4503.
Rabies Vaccination Policy
In accordance with State of Missouri regulations and for the protection of your pet, our staff, and the community, all patients at Rock Bridge Animal Hospital must be current on their rabies vaccination unless medically contraindicated.
Recording Device Disclosure
Rock Bridge Animal Hospital may utilize audio recording devices during appointments to assist in the accurate documentation and transcription of medical records. These recordings are used solely for record-keeping and quality assurance purposes and are handled in accordance with our confidentiality and privacy policies.

Our entire team undergoes training to provide the most positive experience possible for you and your pet.
